Part A: Hospital and Inpatient Deductible and Copays

- Days 1-60: $0 copay for each benefit period
- Days 61-90: $408 copay per day of each benefit period
- Days 91 and beyond: $816 copay per each "lifetime reserve day" after day 90 for each benefit period (up to 60 days over your lifetime)
- Beyond lifetime reserve days: YOU PAY ALL COSTS!

Skilled Nursing Facility Care

- First 20 days: $0
- 21st to 100 day: $204 copay per day
- 101st day and thereafter: YOU PAY ALL COSTS!

Hospice Care

Part B Premium
(Physician and Outpatient Services)

- Must continue to pay Part B premium if enrolled in a Medicare Advantage or Medicare Supplement plan.
If you don't enroll in a timely manner when first eligible, you will need to pay a Part B penalty premium for the rest of your life.

Part B Deductible, Coinsurance and Maximum Out of Pocket (MOOP)

Deductible: $240 per year

Coinsurance: After your deductible is met, you typically pay 20% of the Medicare-approved amount for most doctor services (including most doctor services during an inpatient hospital stay), outpatient services, labs, Part B drugs and durable medical equipment

Maximum Out of Pocket (MOOP): UNLIMITED. For traditional Medicare Part B, there is no annual MOOP for Part B expenses. Therefore, there is no annual limit for the amount of your cost sharing responsibility.

Part D Premium

- Included in Part C premium, if any, if drug coverage is provided through a Medicare Advantage plan.
If you don't enroll within 63 days of losing creditable drug coverage, you will need to pay a Part D penalty premium for the rest of your life.
